What to Expect at Our Heart Valve Clinic

If you or a loved one is dealing with heart valve disease, we’re here to make it easy to get expert care at our Heart Valve Clinic when you need it. There are a few things you can do to prepare for your first visit.

When you contact your valve clinic coordinator, she’ll ask a few questions to get the process started. We’d recommend having your current doctor’s contact information, your medical records and relevant health history available.

For your first appointment, remember to bring:

  • Your insurance card
  • Your photo ID
  • Your co-pay amount
  • Questions for your care team
  • The completed New Patient Form

Your Heart Valve Patient Coordinator

Navigating through medical care can be stressful and complicated. When you or a loved one is dealing with a heart valve issue, getting back to your health should be the focus. You shouldn’t be confused about what services you need or where to go to find them. That’s where our patient coordinators come in.

Your patient coordinator is a highly trained registered nurse who will help with any questions you may have — whether over the phone, via email or in person. They make sure you and your care team are following your care plan and address any concerns you may have. And they can help you better understand medical conditions, medications and instructions.

Your patient coordinator will help you navigate the care process from start to finish. They’ll also help streamline communication with the rest of your care team, including your referring provider.

    Working With Your Patient Coordinator

    Early on in your journey at the Heart Valve Clinic, your coordinator will begin asking about your medical history and review what medical records and documentation we’ll need from you. She’ll also talk to your entire care team to make sure your care plan reflects your wishes.

    Your patient coordinator will also help you schedule follow-up appointments, fill prescriptions and arrange for any special equipment or therapy in your care plan. Your patient coordinator is your partner in recovery.

    Our heart valve clinic coordinators are available Monday through Friday from 7 a.m. to 3:30 p.m.
