Lifelong Care and Education for Congestive Heart Failure
Heart failure is a lifelong condition that can affect your entire body. With early intervention and regular health education, you can navigate and manage heart failure. Our main priority is to help you live a long, happy life.
What is Heart Failure?
Heart failure happens when your heart can't pump enough of the oxygen-rich blood the other organs in your body need. It can develop from either systolic dysfunction (in which the heart is too weak to pump blood effectively) or diastolic dysfunction (in which the heart is too stiff to fill appropriately). It's a serious heart disease. But it's possible to manage your symptoms and slow the progression of heart failure.
Heart failure is usually caused by other conditions such as coronary artery disease, heart valve disease, high blood pressure or unexplained weakening of the heart muscle (idiopathic cardiomyopathy). Your doctor will evaluate your individual case and identify if they can reverse any of your heart damage to improve your symptoms.
In addition to a wide range of treatment options, we provide consistent monitoring and patient education opportunities to improve your quality of life and help you have better health outcomes.
Helping You and Your Loved Ones Understand Heart Failure
We offer educational visits to give you and your loved ones the latest tools so you can take an active role in managing your heart failure. At these visits, your care team will discuss:
- Daily monitoring techniques of signs and symptoms
- Diet and salt intake
- Medication management
- Physical activity
Research shows that reducing unnecessary emergency room visits and hospital admissions is associated with better health outcomes if you’re living with a chronic condition. We have short-notice clinic appointments so you can get support for your urgent heart failure concerns and reduce unnecessary disruptions to your life.
