Heart Failure Overview
At St. Luke's Heart Failure Clinic, we are dedicated to empowering patients with comprehensive care through our Heart Failure Disease Management Program. Our ultimate goals are to enchance quality of life, reduce the need for hospital visits, and empower patients to take control of their heart health, ensuring a proactive path towards wellness.
St. Luke's Heart Failure Clinic utilizes a multidisciplinary team to assess, treat, and manage each aspect of heart failure disease. The heart failure team comprises board-certified heart failure cadiologists, nurse practitioners, disease managers, dietitians, chaplains, social workers, pharmacists, exercise physiologists, and physical and recreational therapists.
