Program Offerings

  • Identifying and Addressing Reversible Causes: We strive to evaluate and treat any reversible factors contributing to heart failure, aiming for stabilization and improvement of heart function.
  • Optimizing Medication: Our team works diligently to ensure that each patient's medication regimen is maximized for the best possible outcomes, tailoring treatment plans to individual needs.
  • Patient Education: We believe in empowering our patients with knowledge. Our program educates patients about the causes of heart failure, recognizing early warning signs and symptoms, and the significance of timely intervention. We also provide guidance on daily monitoring, medication management, dietary recommendations, exercise, activity levels, and available treatment options.
  • Enhancing Quality of Life: Our goal is to maximize the quality of life for individuals living with chronic heart failure, helping them to lead fuller, more active lives despite their condition.
  • Reducing Hospital Visits: By providing comprehensive care and support, we aim to minimize the need for emergency room visits and hospital admissions, and when hospital care is necessary, to reduce the length of stay.
  • Empowering Wellness: We encourage our patients to take an active role in managing their heart failure, equipping them with the tools and knowledge needed to control their wellness journey.

Diagnosis and Treament

Our experienced clinical team at the St. Luke's Heart Failure Clinic strives to provide each patient with the highest quality of care in preventing, diagnosing, and developing personalized heart failure treatment plans. Our aim is to help our patients live better, fuller lives for longer.

The Heart Failure Clinic offers the following:
  • State-of-the-art diagnostic imaging and testing to identify causes of heart failure
  • Individualized treatment plans using evidence-based therapies
  • Access to the latest proven therapies for improving symptoms
  • Informational visits that give patients tools to take an active role in managing their heart failure
  • Guidance on early warning signs and symptoms, daily monitoring, medication management, diet and salt intake, and physical activity
  • 24-hour on-call services to support patients and families
  • Short-notice clinic appointments for urgent symptoms and other concerns
  • Collaboration and communication with primary care physicians, cardiologists, and other specialists
Treatment plans are developed through a thorough evaluation and conversations between the patient and our heart failure clinicians. This may involve:
  • Diuretics or "water pills" for symptom management
  • Evidence-based medications to stabilize and improve heart function
  • Devices that may be appropriate, such as biventricular pacemakers and ICDs
  • FDA-approved therapies to improve symptoms in advancing stages of heart failure
  • Devices that aide in the early detection and treatment of heart failure exacerbations
