Home Health

St. Luke's provides patients with the care they need in the comfort of their home when their physician orders home health as a medically necessary part of recovery. The St. Luke's Home Health team works closely with the patient's physician to develop a plan to provide care for the patient at home. Home care offers a full range of nursing, therapy, social work, and aide services. The home nurse works with each patient on comprehensive care plans including assessments, teaching, treatments and therapies needed for recovery. Your Treatment Plan
St. Luke's Home Health Services will:

• Educate you about your disease process or condition, medication safety, nutritional needs and infection control.
• Assess your pain and symptoms, fall risk and social and economic needs, as well as the safety of your home environment.
• Provide families and caregivers with education about care during recovery and safety tips on how to prevent falls.
• Provide services including wound or ostomy care, catheter care, tracheotomy care, pain management, diabetic care and assistance with personal care.
• Provide therapies to help you safely use assistive devices and adaptive equipment as well as regain your mobility and independence.

Admission & Fees
You can be admitted to St. Luke's Home Health Services only under the direction of your physician. The admission is based upon needs of the patient, homebound status and types of services required.

St. Luke's Home Health Services provides care for individuals in St. Louis City and County, St. Charles County and Jefferson County. Most services are covered by Medicare and/or private insurance. If any services will not be covered under Medicare or your insurance, we will discuss the fees with you before providing the services. You can pay directly for your services if you are not covered by Medicare or another insurance plan.

St. Luke's Home Health Services is licensed by the Missouri Department of Health and Senior Services and certified by Medicare as an approved provider of Medicare home health services.
