Self-Registration

Don't miss out on the ease of the mystlukes patient portal.

Signing up is simple!

1. To enroll online (if current patient and 18 or older), visit the self-enrollment pageopens in a new tab.
2. To enroll via a form, download the form hereopens in a new tab, then mail or fax to the information on the form.
3. To enroll in person, visit your physician's office and follow these steps:
   • Provide email address and security question with answer.
   • Follow the sign-up steps after receiving an enrollment confirmation email (valid for 90 days).
   • Log into the website hereopens in a new tab or download the app on Apple or Android mobile devices.

Family Member or Dependent Access

You can download our proxy authorization form if you need access to a family member or dependent's mystlukes patient portal.

• Family member or dependent must be 13 years old or older and willing to agree to provide access by signing the form.
• Family member or guardian will not have full access to the dependent's mystlukes patient portal.

To gain proxy access in person, accompany the family member or dependent to their physician's office so they can provide verbal consent for you. No form is necessary.
