St. Luke’s Hospital (“COMPANY”) is committed to protecting your privacy. This Privacy Policy explains our information gathering and dissemination practices. If you have questions or concerns regarding this Privacy Policy, please contact COMPANY at the email address provided at the end of this Privacy Policy.

This Privacy Policy applies to information COMPANY collects on the www.stlukes-stl.com (the “Site”), including in email, text, and other electronic messages between you and the Site. It does not apply to information collected by COMPANY offline or through any other means, including on any other website operated by COMPANY or any third party, including through any third-party applications or content that may link to or be accessible from or on the Site.

Please read this Privacy Policy carefully to understand COMPANY’s policies and practices regarding your information and how COMPANY will treat it. This Privacy Policy supplements the Notice of Privacy Practices (“NPP”) available here which governs our access, use and disclosure of your protected health information, as well as our Website Terms and Conditions available here. 1. WHAT INFORMATION IS COLLECTED.

We collect certain personal information from you in certain areas of the Site for various reasons, including information that identifies you or which could reasonably be used to identify you, whether actively submitted by you or automatically-collected through the Site.

COMPANY collects personal information in two main ways:

Personal Information You Submit: COMPANY collects any personal information that you enter into data fields on the Site. For example, you may submit your name, postal address, e-mail address, and/or other information in order to receive information about various subjects, register for programs, contact customer service, or respond to surveys.

Automatic Collection of Information: COMPANY may collect information automatically about your visits to the Site without you actively submitting such information. Information may be collected using various technologies, such as cookies, Internet tags, and web beacons. Like many other websites, COMPANY makes use of log files. The information inside the log files may include internet protocol (IP) addresses, type of browser, Internet Service Provider (ISP), date/time stamp, referring/exit pages, and number of clicks. COMPANY uses this information to analyze trends, administer the Site, track user’s use of the Site, and gather demographic information. Some features and content on the Site that are served by third-party vendors may also automatically collect your personal information. For example, COMPANY uses Google APIs to display user locations and maps, so you can find a doctor near you or navigate to COMPANY locations. COMPANY may collect Google user information, such as geolocation data, for usage in conjunction with Google APIs when you request such services. COMPANY will only request access to the Google user information and permissions necessary to implement the relevant services. The use of Google APIs is subject to the Google Privacy Policy.

COMPANY may use certain tracking technologies to collect and analyze information internally about how users interact with our website as part of our health care operations.  We do not use tracking tools for targeted marketing.  No third party tracking technology vendor has access to protected health information unless it has a business associate agreement binding it to maintaining the privacy and confidentiality of such information or except as otherwise permitted by HIPAA.

The following is a further description of certain information that may be collected through the Site:

Registration Information. Registration may be necessary to use certain areas of the Site. When you register at the Site, we ask you for certain personal information, such as name, e-mail address, mailing address, and phone number. All of this information is used to deliver a more personal and pertinent experience for registered users of the Site.

IP Addresses. An IP address is a number assigned to your device when you connect to the Internet. As part of the protocol of the Internet, web servers can identify your device by its IP address. In addition, web servers may be able to identify the type of browser and/or type of device you are using. COMPANY collects IP addresses and related information for the purposes of system administration, to assess the traffic to the Site and to maintain and improve the Site. We do not actively link IP addresses to your other personal information as a general matter. We can and will use IP addresses, however, to identify a user when we feel it is necessary to enforce compliance with our Website Terms and Conditions or to protect our service, the Site, customers or others, at our sole discretion.

Use Of Cookies. A cookie is a small file placed on the hard drive of your computer. COMPANY uses cookies to analyze the traffic, such as total visitors and pages viewed on the Site, and make improvements based on the data we collect. COMPANY never saves passwords or credit card information in cookies. Most browsers are initially set up to accept cookies. You can usually set your browser to refuse cookies or indicate when a cookie is being sent. If you refuse cookies, you may not be able to use some of the functionality of the Site. COMPANY collects the information described above via the Site. 3. HOW COLLECTED INFORMATION IS USED.

In General. COMPANY uses personal information to learn more about how visitors use the Site and to measure areas of interest. This allows COMPANY to build features and functionality that will enhance your Site experience. We also may use personal information to filter out those who do not comply with our Website Terms and Conditions. If you so choose, personal information might also be used to send you information about new features and services on the Site or offered by COMPANY or to send you information from selected third party product or service providers. We may also disclose certain information in court documents if we bring an action against you, for example, to collect a debt you owe us.

You may sign up to receive COMPANY’s newsletter or to receive information about new features, events, offers and special coverage on the Site and about special offers from COMPANY’s partners. If you do not wish to have your contact information used by the COMPANY for these purposes, you can opt-out using the unsubscribe mechanism contained in the communication you receive or by sending COMPANY an email stating your request to webmaster@stlukes-stl.com. This opt out does not apply to information provided to the COMPANY as a result of a product or service experience or other transactional matters.

It is important to know that you may inadvertently provide personal information to third parties through your postings to a forum or participation in a chat session, and we have no control over their use of the information you may provide. Please keep in mind that whenever you give out personal information online (for example, in forums, message boards or chat sessions) that information can be collected and used by people you don’t know. While we strive to protect our users’ personal information and privacy, we cannot guarantee the security of any information you voluntarily disclose online, and you do so at your own risk.

6. CHILDREN UNDER THE AGE OF 13 YEARS.

In accordance with the Children’s Online Privacy Protection Act (COPPA), COMPANY does not intend to, and will not knowingly, collect or accept personal information from a child under the age of 13 without prior consent of a parent or guardian. If we become aware of a child under the age of 13 providing personal information without the prior consent of a parent or guardian, we will attempt to delete that information. If your child has provided personal information without your consent, please contact us so we can delete it. If you are under the age of 13, do not provide us with any personal information until the consent of a parent or guardian has been confirmed with COMPANY.
