Diabetes Exercise Program
St. Luke's Wellness Center offers the Diabetes Exercise Program (DEP) for active or relatively healthy individuals with diabetes or pre-diabetes. This program is ideal for those who desire an independent home or gym-based program designed specifically for their needs. Those who have had a cardiac event in the past year or have a history of heart failure may be referred to outpatient cardiac rehab prior to joining the DEP. Intensive Behavioral Therapy (IBT) for Weight Loss
A personalized weight loss program for individuals with a BMI greater than 30 that focuses on behavior and lifestyle changes. Participants work one-on-one with a registered dietitian up to 22 visits in one year.
Program is covered by Medicare with no deductible. If not covered by Medicare, we will work with you to verify your insurance coverage. A physician's order is required to bill insurance. Self-pay options are available.
St. Luke's Hospital Nutrition Wellness and Diabetes Center Medicare Diabetes Prevention Program (DPP) is a Medicare-covered lifestyle change program for participants 60 and older to help lower the risk of type 2 diabetes. This program is backed by years of research and recognized by the Centers for Disease Control and Prevention and is proven to lower the risk for type 2 diabetes among participants who follow the program and lose 5 - 7 percent of their body weight.
Participants will meet in a group setting for 16 core sessions over a six-month period. The sessions cover a number of topics including making lasting changes around diet and exercise, and strategies to control your weight. Once you complete the core sessions, you'll receive six monthly follow-up sessions to help you maintain your healthy habits.
