A baby’s birth is one of life’s most special events. St. Luke’s “Footprints” wall is a wonderful way to commemorate the birth of your child, grandchild, niece or nephew.

Each Footprint plaque is uniquely crafted and inscribed with the baby’s name and birthdate. (Plaque dimensions are 3 ½” x 4” and gift amount: $200)

Your Footprints contribution is tax deductible and provides support for St. Luke’s Maternity Services’ education materials and classes for expectant parents.

Please complete the online form below, or print off the attached form and return it to St. Luke’s Hospital Office of Development along with your contribution. You will be notified when your plaque has been installed on the Footprints wall, located on the 5th floor of the hospital.
