St. Luke’s commitment to improve the health of the communities we serve, as reflected in our mission, is facilitated through the development of Community Health Needs Assessments (CHNA) and Community Health Implementation Plans (CHIP). The process to develop the CHNA and CHIP helps us to better understand and focus on the important health and health-related issues affecting our communities and to develop strategies and implementation plans intended to drive meaningful results.
